It's more an informal thing. There is not really a button saying 'Create a clan'. It's more like people change their name to something like [Clan]_Kris or something similar, make a Skype group and go play minigames together.
OITC clans are basically a group of people that play OITC together and help each other improve. Clans (once more are made) also can do events like 1v1 tournaments of Clan vs. Clans and stuff like that. How to get in or make one? All you have to do to get in one is to apply for that clan. There are many clan forums here, so find one that you like, and apply. If you want to make one, all you have to do is think of a clan name and create a thread.

Personally, I am not really into clans. I made one for the worst OITC players, but I am not the one to suggest things about OITC.
